My 23 year old son recently built a skateboard - bmx bike half-pipe / ramp in his grandparent’s yard.
They (the grandparents) are concerned about the liability of someone other than a family member riding on it and getting injured.
I found a statement of liability waiver – consent form online for prospective users to sign indicating this is done at their own risk and would not hold the property owner liable for possible injuries up to and including possible death.
It is to be signed and notarized by the individual (plus parent / guardian if under the age of 18) who wishes to ride on this.
This would be no different than having a swimming pool, trampoline, etc. on the property.
Is this a binding and legal document that would absolve any claims resulting from personal injury?

Here are two articles I found that explain the risks of having something like a skateboard ramp or trampoline in the backyard. There is definitely the potential for accidents to occur when teens and young adults use these kinds of equipment, and they should always wear protective equipment like helmets and pads to minimize the risks of serious injury.
As far as the waiver goes, I think you would have to ask a lawyer or someone from your insurance company to make sure that would stand up in court if someone got hurt.
